National System for Incident Reporting – Radiation treatment pilot assessment report, Summary

The National System for Incident Reporting (NSIR), launched by the Canadian Institute for Health Information (CIHI) in 2010, is a voluntary, web-based reporting system that collects medication incident data from acute and long-term care facilities.

CPQR-endorsed measures for patient-reported outcomes

CPQR supports the consistent use of PRO in Canadian radiation treatment programs to improve the clinical encounter for patients. Promoting the use of the same Patient Reported Outcome Measures (PROM) across the country will also help facilitate big data analysis against treatment outcomes which may be used to inform population-based decisions regarding optimal care.

The Pan-Canadian Collaboration for Quality Radiopharmaceutical Therapy (PCQRT) underscore crucial elements of radiopharmaceutical therapy quality assurance that should be consistent across all Radiopharmaceutical Therapy programs in Canada. They do not aim to supplant detailed specifications, standard operational procedures, or institution-specific policies. Instead, their purpose is to support the formation and preservation of a national approach to radiopharmaceutical therapy quality assurance.
